Why normal iron results can sit alongside iron symptoms

Haemoglobin measures iron currently in circulation. Ferritin estimates what is held in reserve. Reserves can run low well before circulating levels fall, which is one reason a standard blood count can come back unremarkable while symptoms persist.

A full blood count is the test most often ordered when someone reports fatigue. It reports haemoglobin, which reflects the iron already committed to carrying oxygen in red blood cells. If that number sits inside the reference range, iron is frequently considered ruled out.

The body holds iron in more than one place. Beyond what is circulating, a reserve is stored in the liver, spleen and bone marrow, bound to a protein called ferritin. A small amount of ferritin leaks into the blood in rough proportion to the size of that reserve, which is what a serum ferritin test estimates.

The reserve is drawn on first. When intake falls short of losses over months, storage depletes while the body continues prioritising haemoglobin production. This produces a stage where ferritin is low, haemoglobin is still within range, and the standard blood count looks unremarkable. Symptoms reported at this stage often include fatigue, reduced exercise tolerance, hair shedding and cold intolerance, though none of these are specific to iron and each has many other plausible causes.

Reference ranges add a second complication. Lower limits for ferritin vary considerably between laboratories, with some setting the floor as low as 10 or 15 micrograms per litre. A good deal of clinical literature suggests symptoms can occur at levels above those thresholds, and some guidance uses 30 as a more conservative marker for depletion. A result flagged as normal may therefore be normal by that laboratory's cutoff without being optimal for you.

One caveat worth holding. Ferritin is an acute phase reactant, meaning it rises during inflammation and infection independently of iron status. A ferritin drawn during an infection, a flare of an inflammatory condition, or shortly after intense exercise may read higher than the underlying reserve. Where inflammation is suspected, ferritin is usually interpreted alongside CRP or transferrin saturation rather than alone.
